Offer exclusive to the first 114,866 Ontario resident AIR MILES Collectors who register for, complete and submit the Ontario Health Study’s online questionnaire or by 11:59:59 p.m. on January 20, 2012 whichever occurs first and who follow the online instructions to enter a valid AIR MILES Collector Number by February 29, 2012.

Offer is not available to those AIR MILES Collectors who have previously registered for, completed and submitted the Ontario Health Study’s online questionnaire.
